50 Minute Children's Initial Consultation Please fill out the Registration Form and Food Diary and sent back prior to appointment.

Cancellation Policy

To cancel or reschedule, please email or contact me 24 hours in advance as we are a busy clinic and it is important each person gets the chance to book if a cancellation occurs. I understand emergencies can occur, but reoccurring failure to show or cancellations with less than 24 hours notice will incur a cancellation fee of 50% of the appointment fee.
